FMLA (Family & Medical Leave Act) requires CARC to provide eligible employees up to 12 weeks of unpaid, job-protected leave in any 12 month period for certain family and medical reasons. Employees are eligible if they have worked for CARC for at least one year and 1,250 hours over the previous 12 months.

• For incapacity due to pregnancy, prenatal medical care or child birth.
• To care for the employee's child after birth, or placement for adoption or foster care.
• To care for the employee's spouse, dependent son or daughter, or parent, who has a serious health condition.
• For a serious health condition that makes the employee unable to perform the employee's job.

Employees must provide medical certification from a healthcare provider for CARC to determine if the leave qualifies for FMLA protection and the anticipated timing and duration of the leave.

Please Note: The FMLA "Certification of Health Care Provider" form must be completed and returned to the CARC Personnel Office. This is the first step in applying for FMLA leave.
